Appointment Info + Forms Page

The St. Francis Center for Surgical Care is taking appointments. If your insurance carrier allows self-referrals, you may contact St. Francis Center for Surgical Care directly for appointments without a referral from another doctor. To schedule an appointment with one of our physicians, please have your current insurance card, social security number and basic demographic information available. If your insurance carrier requires a referral from your primary care physician, please schedule your appointment through your primary care physician.

To schedule an appointment with a physician at the St. Francis Center for Surgical Care, call 706-324-3243.

Patients are seen by appointment only, and we respectfully request that you notify us well in advance if you are unable to keep an appointment. This will enable us to give this appointment time to another patient.

All previous studies must be brought to your appointment. Please contact any other treating physicians so that they may promptly fax any pertinent medical records to our office at 706-324-3835.

Your insurance information and demographic information are requested when your appointment is made, so that we may verify your coverage, deductibles and policy guidelines. Failure to provide this information will deem you a self pay patient and payment will be due at the time of service.

Office Hours: Monday thru Thursday 8:30 a.m. to 4:30 p.m.
Friday from 8:30 a.m. to 12:30 p.m.

There is a physician on call 24 hours a day, seven days a week. Please call 706-324-3243 and the answering service will contact the physician on call.

Your Role and Responsibility as a Patient

If your insurance requires that you have a referral from your primary care or referring physician, it is your responsibility to secure this. If you come for an appointment without it and are seen you will be deemed as self pay. You are responsible for deductibles, co-pays and non-covered charges pre-operatively. Should you be uninsured, we will make every effort to work out arrangements suitable to you and your physician.
